50 units of accommodation for families and single people as an alternative to hotels and hostels, using traditional housing settings. Providing holistic needs assessment, brokerage and referrals to support access to health, treatment and housing pathways to sustain a permanent exit from homelessness.
22 units of accommodation (Carlow & Kilkenny) through the recovery approach with life skill supports, and tenancy sustainment interventions for people with a mental health diagnosis, working as part of a multidisciplinary approach.
Twelve participants of people vulnerable with a mental health diagnosis, with significant challenges to managing a home, inclined to hoarding and other high-risk behaviours, vulnerable to losing their tenancy. Providing care and case management, healthcare supports and cleaning. Working in conjunction with Mental Health Multi-Disciplinary services.
Providing Tenancy Sustainment supports and access to GP, mental health supports for local authority and approved housing body tenants at risk of homelessness.
Two distinct models of service:
A High Support Housing for people at risk of repeated homelessness due to mental health, addiction and dual diagnosis. Providing care and case management services, healthcare supports and cleaning. Working in conjunction with local health and community services.
B Tenancy
Support to 200 households across Carlow, Laois, Kilkenny and Tipperary. Who are
elderly, people with disabilities, families with an experience of and/or at risk of
homelessness.
